Title: Improve approval button UX in Copilot Workspace — Fix scroll depth issue

Description:

The approval button in Copilot Workspace is buried deep in the scroll area, forcing users to scroll down even after reading the generated code. This creates friction in the review workflow.

Current behavior:

1. Copilot generates code
2. User reads the changes
3. User must scroll down to find the approval button
4. Button appears at bottom of page

Expected behavior:

1. Approval button should be sticky/fixed at top or inline with the code preview
2. User should approve without unnecessary scrolling

Impact:

• Approval friction increases review time
• Users develop habit of NOT reviewing thoroughly (scroll fatigue)
• Reduces Copilot adoption in review-heavy workflows

Suggested fix:

• Add sticky approval button toolbar (similar to GitHub PR review interface)
• Or: Place approval button at top of generated code block
• Or: Add keyboard shortcut (Cmd+Enter to approve)

This is a small UX polish that would significantly improve daily workflow efficiency.
