Should explicitly describe that manual creation of Jumpbox VM is required

This document and GitHub workflow don't mention that a jump box VM is not created, though Azure Bastion is deployed. It is okay to create the VM manually for security reason, but I suppose that explicit description (for example, when you need a jump box VM, you have to deploy it manually) should be added to the README.md.
