What steps does it take to reproduce the issue?
In a sub collection create a Template. After giving the Template a name and adding default metadata items click "Save + Add Terms" button. Instead of display the Terms of Use and Access page for inputting default values the cursor returns to the top of the page. A second click of the "save" button opens the Terms Page, but there are no fields for default value entry. The Template is actually saved with the previously entered default values and you can go back into the template to Add/or edit default terms from the Manage Templates page.
