connection is not closed when process is terminated

If you close a debugger the page still shows as loading. This is presumably
since the removal of the ProcessMonitor which would have resumed the
original process when the second process was terminated.

I'm guessing we need an #ensure: or #ifCurtailed: or something in the
server adaptor...
```
