Create a way to donate audio samples using a web form

On the plot form, add "donate audio sample". It uploads to a server (where?) and then opens a google form to add information to that sample:

• Speaker demographics
• Speaker accent / disabilities
• Intended word
• Intended vowel sound (make a huge dropdown)
• Contact information
