- remove notebook 5.0.0rc1 travis build, since notebook 5 is now default
- avoid problematic conda versions on travis
Since it can't actually install anything because it throws TypeError versions 4.3.14 through 4.3.16 are affected. See conda/conda#5033 for details.
for some reason, the readthedocs sphinx ext doesn't seem to register correctly on sphinx >=1.6, presumably related to the transition to using entry points. For now, just use <1.6
