[BUG] Duplicate buttons in monitor page

What is the bug?
Actions and Create monitor buttons are duplicated in monitors page. Not an ideal UX to have duplicate buttons in the same page.

How can one reproduce the bug?
Steps to reproduce the behavior:

1. Go to monitors page

A clear and concise description of what you expected to happen.

• Plugins: alerting
