More changes for pthread threading support#10

This follows the 3dbrew.org wiki naming. Also added a get version.
In the case of the main thread we use a hack since there's no nice API for getting the main thread's handle (see code).

Implements some more pthread functions for Meziu/rust-horizon#10.
Requires Meziu/libc#7.
The first commit I actually forgot to push in the last PR, but the rest of the changes are generally to use more standard interfaces.
